    Geriatric & Palliative Medicine Mini-Fellowship for Non-Geriatricians
    We are pleased to announce Mount Sinai School of Medicine's annual Physician Mini-Fellowship: Geriatrics for Non-Geriatricians -- How to Integrate Geriatrics into Your Curriculum and Teaching. The course will take place Monday, March 3 to Wednesday, March 5, 2008 in New York City. It is our hope that this information as well as the pdf of the application and flyer below can be included in your website publication's announcement section as well as in list serves or newsletters that may be appropriate. The free, three-day Mini-Fellowship is targeted at non-geriatrician physicians responsible for teaching medical students, residents, or fellows how to care for older adults. The course covers geriatric content, palliative medicine, advanced teaching skills, curriculum development, and the geriatrics approach to patient care. Sessions are taught through demonstrations, self-study, small- and large-group sessions, e-learning, role-playing, and post-course mentoring. We offer four clinical tracks and two teaching tracks. CME credits are awarded. The course is free of charge for physicians affiliated with a US medical school or teaching hospital, who have direct teaching responsibilities with medical students, residents, or fellows, and are non-geriatricians (may not have sat for the geriatrics boards or have completed, or be enrolled in, a geriatrics fellowship).     Functional MRI chronic pain research study
    This is a NIH-funded neuroimaging study that aims to teach chronic pain patients cognitive strategies to manage their pain by controlling brain activity in pain-related regions of the brain. We are currently inviting chronic pain patients to participate in a study to investigate a treatment for pain that uses newly developed brain imaging methods to train participants to control their own brain activation. All participants in this study will receive a new form of cognitive training for controlling pain. A central goal of the study is to measure the impact of a new form of cognitive strategies for pain control on activations from brain regions involved in the experience of pain. Participants will be scanned using functional MRI while they employ cognitive strategies allowing the participant to view their brain activity. Some participants will see their own brain activation during this process while other participants will see very similar but simulated data that does not come from their brain. However, a Canadian advocate for chronic pain sufferers is coming to Hamilton next week from Halifax, intent on showing chronic pain sufferers in this area that banding together and offering each other support and advice is an effective way to improve their quality of life. Terry Bremner is the speaker at a public forum on Tuesday, Aug. 9, that will give those affected by chronic pain the chance to share their experiences and learn how to launch their own support group. The session is being held at 7 p.m. at the Royal Botanical Gardens, 680 Plains Road West.

    A Position Statement from the American Pain Society
    Chronic pain is a significant problem in the pediatric population, conservatively estimated to affect 15% to 20% of children (Goodman & McGrath, 1991). Children* and their families experience significant emotional and social consequences as a result of pain and disability. The financial costs of childhood pain also may be significant in terms of healthcare utilization as well as other indirect costs, such as lost wages due to time off work to care for the child (Li & Balint, in press). In addition, the physical and psychological sequelae associated with chronic pain may have an impact on overall health and may predispose for the development of adult chronic pain (Campo et al., 1999; Walker, Garber, Van Slyke, & Greene, 1995).

    CHRONIC PAIN STUDY
    Because the understanding and management of chronic pain is complex, and because it has been an unsolved, difficult problem for all involved, this report was requested. No one will be surprised that we will not provide simple answers to the long standing questions that have not had solutions. Rather, we will provide a reasonable understanding of chronic pain, so that reasonable policies can be based on current concepts and understanding. I have tried to be as objective as possible, taking the stance that I am on no one's "side" in this matter. Although commissioned by the Workers Compensation Board of Nova Scotia, I have tried to approach the problem of chronic pain as a clinical issue, leaving the policies that might result to others.

    New Target Identified for Chronic Pain Therapy
    Scientists funded by the National Institute of Neurological Disorders and Stroke (NINDS) may soon be able to reduce sensitivity to stimuli that are associated with chronic neuropathic and inflammatory pain by disabling certain nerve cells that send pain signals to the brain. Research reported in the November 19, 1999, issue of Science shows that, in an animal model, combining substance P with the ribosome-inactivating protein saporin (SAP) will inhibit the pain associated with nerve injury when administered before or after the development of neuropathic pain and will significantly reduce sensitivity to stimuli associated with inflammatory pain. Previous data have suggested that in the peripheral nervous system, neuropathic and inflammatory pain arise from different mechanisms and are conveyed to the spinal cord by distinct groups of primary afferent neurons. Study findings suggest that these different types of pain conditions, which can arise via different mechanisms, may be conveyed to the spinal cord by the same substance P receptor-expressing neurons. Further, the study found that, following substance P-SAP treatment, opiates such as morphine remain a viable therapy for breakthrough pain. This discovery could lead to new treatments for chronic pain and the development of more targeted pain-relieving drug therapy.

    Study Links Chronic Pain to Signals in the Brain
    For centuries, doctors have tried to find effective ways to treat chronic pain, a devastating neurological disorder that affects almost 90 million Americans. A new study shows that two proteins in the brain trigger the neuronal changes that amplify and sustain this type of pain. The finding may lead to new ways of treating chronic pain. "This is the first [chronic pain] study to show clear molecular targets in the brain," says Min Zhuo, Ph.D., of Washington University in St. Louis, Missouri, senior author of the report. "Drugs that inhibit these two proteins may help to reduce chronic pain." The study was funded in part by the National Institute of Neurological Disorders and Stroke (NINDS) and appears in the November 14, 2002, issue of Neuron. 1

    Coping with pain produced by physical activity in persons with chronic low back pain.
    Coping with chronic pain during exposure to pain produced by activity was examined in 30 patients with chronic low back pain referred to a university pain management center. Patient's range of motion, autonomic responses, and anticipatory anxiety ratings before exposure and ratings of pain and anxiety after exposure were assessed, and the number of repetitions of the activities that produced the pain was recorded. Analyses showed that using coping self-statements was associated with lower skin conductance during anticipation and greater range of motion. Praying, hoping and catastrophizing were associated with greater anticipatory anxiety, greater anxiety during the painful activity, and less range of motion from the onset of increased pain to the point of pain tolerance. Praying and hoping were associated with higher pain ratings and fewer repetitions of the activity. Assessment of coping during an incident of pain and multiple methods to measure pain and distress provided convincing evidence that patients' self-management responses influence the consequences of pain exposure.
